Participating insurance

We are a preferred provider for most area insurers including 1st Nation/Tribal Health, Aetna, BCBS, Cigna, Great West Health, Medicare, Medicaid, Meritain, TriWest, VA, TriCare, Wise Network, and Wyoming Worker’s Compensation. We’ll work closely with you to ensure the most efficient, cost effective treatment possible.

ATTENTION: UnitedHealthcare will no longer be an accepted in-network plan for Elevate Rehab as of January of 2024. This includes any UnitedHealthcare Medicare Advantage plan. United plans that are secondary or Medicare Supplement plans will be honored. We truly apologize for any inconvenience.

What to Bring to Your Appointment

Physician referral or prescription for therapy if required by your insurance.  Wyoming is a Direct Access State, which means physical and occupational therapists may treat based on medical necessity but your insurance may require an order to be covered. Call us to help you determine your specific needs.

Your insurance card or claim number in order for Elevate Rehab to bill the insurance on your behalf for services delivered.

Loose, comfortable clothing appropriate for your diagnosis: tank top or swim suit if shoulder or upper back, shorts if hip or knee, tennis shoes if lower extremity, short sleeved shirt if elbow or upper arm, etc.

Orthotics, splints or braces that you are currently using or have used to manage your symptoms, whether the device was successful or not.

Postoperative/surgery precautions/home exercise instructions provided by your MD or previous therapist.
